Terms and Conditions of Service

Welcome to Phlebotomist Express LLC. By booking our mobile phlebotomy services, you agree to the following terms and conditions:

1. Services Provided

Phlebotomist Express LLC provides mobile blood collection and specimen delivery to accredited laboratories. We are not a laboratory and do not perform the testing/analysis ourselves. We act as an independent mobile collection service.

2. Patient Preparation

It is the patient’s responsibility to follow all pre-procedure instructions provided by their physician (e.g., fasting for 8–12 hours, hydration). If a procedure cannot be performed because the patient failed to fast or follow instructions, a service fee will still apply.

3. Appointment & Cancellation Policy

  • Cancellations: We require at least 24 hours’ notice for cancellations or rescheduling.

  • Late Fees/No-Shows: If the phlebotomist arrives at the location and the patient is not present or refuses service without prior notice, a “No-Show” fee of $[Insert Amount] will be charged.

4. Lab Orders and Results

  • A valid physician’s order is required for all collections.

  • Results: Phlebotomist Express LLC does not provide medical results directly to patients. Results are delivered by the laboratory to the ordering physician. Please contact your doctor for your results.

5. Limitation of Liability

While our phlebotomists are certified and follow strict clinical protocols, the patient understands that minor bruising or discomfort is a normal risk of venipuncture. Phlebotomist Express LLC is not liable for issues arising from the laboratory’s analysis or delays in lab processing.

6. Payment and Insurance

  • Our service fee is for the mobile collection and transport only.

  • This fee is usually not covered by insurance and must be paid at the time of booking or service.

  • Laboratory testing fees are separate and are typically billed by the lab to your insurance provider.
